ADHD and ASD referral forms
Useful information regarding the new Neurodevelopmental Pathway referral process.
If you are requesting ASD and, or ADHD assessments for children aged 5 through to the age of 18 please use the 'tree' form.
When making a referral please submit all relevant supporting information and complete a summary of the child’s presentation (useful information to support an ASD and, or ADHD referral sheet). It is preferable for this to reflect the child both at home and school. Remember, there don’t have to be issues in school, it can be a record of observed differences in presentation compared to their peers.
There is an expectation that the vast majority of referrals will be received from schools (rather than GPs or paediatricians) as they hold a significant amount of information about the child and it is imperative that we work quickly and efficiently with the schools to ensure the child or young person’s needs are being met whilst they are awaiting an assessment.
Children and young people who are electively home educated can access the pathway. The expectation is that families work in partnership with the Education Inclusion team or EHE team to complete the referral form. A referral process will be established in each of the localities.
The process has also seen the introduction of consultation slots being made available to schools for cases that they are unsure of or when experiencing parental pressure to make a referral. The form needs to be signed by the referrer and the child’s parent or guardian for children at primary school. It also needs to be signed by the young person for secondary and post-16 age requests.
The tree form can only be used for the Neurodevelopmental Assessment Pathway.
